Replicate Form

Instructions

1) The Replicate Form link is located in the right-hand column of the form builder page, as shown below. It is used to copy, or re-purpose forms across accounts and studies you are a member of. 

2) The Replicate Form opens the Replicate Form page which is shown in the figure below.

The Replicate Form page allows a user to replicate or copy a form to a new study (or a new version, if the form selected is a Subject type form). If a Subject type form is selected from the Source Host/Study/Form dropdown list, a Version drop-down also appears as shown in the figure above. The desired version of the form to be replicated or copied must be selected from the dropdown list of choices.

If the user belongs to more than one Website Host and study, the forms can be copied across Website Hosts as well. Replicating a form overwrites the form selected in the new destination (i.e., if a Form is selected from the Form dropdown list in the Destination Host/Study/Form section, that form will be overwritten when the function is complete); copying the form makes another “copy” of the form in the new destination (i.e., if no Form is selected from the Form dropdown list in the Destination Host/Study/Form section, the form will simply be copied to the study when the function is complete).
